He had severe cough for the last few hours. He is a known case of Diabetes mellitus, IHD, COPD due to asthma, CKD with much more metabolic complications. When I examined him for the complaints of cough my intention was to find out what was the cause of cough. In such a situation the cough could be arising because of lung infection or because of heart failure. If it is heart failure, it is much more a serious condition and you may lose the patient if not treated urgently. You may also have to assess the stage of the problem like impending heart failure or very chronic heart failure or an acute severe heart failure. CLINICAL CASE NUMBER 5 In our post covid program at Dempo Healthcare over the last 6 months, I have been analysing and treating a large number of post covid patients. Many of them come with a large number of complaints from cough, breathlessness, chest pain, inability walk, loss of memory, loss of appetite, loss of weight, muscle weakness and many more. Infect any system can get affected in covid. Some of them have come with so called long covid symptoms. My first step is to assess them thoroughly right from skin to endocrine functions. Most of these complaints do not worry me. High dose Vit c, Vit-D and other supportive measures set these patients to normally within a weeks’ time. Their CRP/ESR settles down to normal within 4 days. Even the thyroid function improves within a months’ time without adding any thyroid drugs. I need to use my expertise with these patients in analysing and assessing their cardiac status. In 30% of these patients I have seen, have heart involvement.

Raghuvir (name changed) was my patient for the last 2 years. He suffered from Diabetes, IHD,  Kidney failure, Hypertension, congestive cardiac failure. After a meticulous management the patient was stabilised and was leading a good quality life. A regular follow up made him loose about 12 kg of his weight. Everything was stable and good.             Three months back his son gets married with the arrival of a daughter- in-law who is welcomed by the family creating very happy atmospheres in the family. For some time everyone has forgotten father’s sickness. The daughter -in -law takes charge of the house. Today I was shocked to see yet another case of bedsores. A 66 years old female having a healthy life, had not even visited a doctor for any complaints for the last 15 years, was admitted to a good city hospital for mild breathlessness.   She had IHD with mild cardiac failure. She was managed very well and sent home after 10 days of hospitalization. Patient was brought back on a wheelchair while she had walked to the hospital. Seven days had passed at home. She stopped eating and was yelling and the relatives did not know what to do. As there was no support from the hospital for home care, I was called in. I went to see the patient with a nursing staff.
